How to use this form

This form enables you to apply to three medical faculties in the Czech Republic. Once you have submitted this form, Medical Doorway will be in touch to arrange your entrance exams.

Dates of entrance exams are fixed and there is a limited number of seats for each exam.

If there is any issue with availability of seats on any exam, a member of the Medical Doorway team will be in touch to discuss the options open to you.

Once paid, all exam and application fees are strictly non-refundable
